Virtual/Telehealth (Video) or In Person Visits?
As many of you know, the video visit has become very common in the past several years. At Wise-Minded Wellness we can schedule a video visit (live face-to-face session). This option can only be for your Free 15-minute Consult, Initial Consult Appointment and Follow up Appointments. As part of your care, a physical exam is required. This will be arranged sometime after your initial visit and/or as indicated.
We can also do Phone Visits only (No Video); however, this is only for established patients, when no concerns are present.
Of course, in person visits are also welcomed in Stillwater, Minnesota! This is currently in
process, so stay tuned for updates!!
Wise Minded Wellness requires a valid credit card on file to confirm all bookings.
How to Prepare for My First Visit?
Once you are scheduled, you will be sent a Welcome E-mail with New Patient Intake and History Forms! These forms are necessary to start your care and begin your journey with us at Wise Minded Wellness! You will be able to complete the forms online and send back to us. The medical history form is very detailed and comprehensive. It includes past/current symptoms, concerns, medical/surgical/ family/social histories, medications/supplements/treatments tried current and/or prior, etc.… If for some reason you cannot do the forms online – you may print them and bring completed with you to our first appointment (if applicable).
We really appreciate your time and know completing yet another history form(s) can feel daunting. Just know this will be essential for our work together. So please complete and send back the forms ASAP after scheduling – so they can be reviewed prior to our appointment. This allows time to focus on your symptoms, concerns, education and a plan.

To call or reschedule, please contact Wise-Minded Wellness at least 24 hours prior to your scheduled appointment. A 24-hour cancellation notice is required for all appointments. Late cancellations (less than 24-hour notice) are subject to a $50 cancellation fee which will be applied to your credit card on file.
Missed appointments (“No Call/No Show”) will be charged for the entire cost of the visit to the credit card on file.
Helping you is important, I can be flexible and understanding, but please respect my time by giving advanced notice if you plan on missing your appointment.
Do you accept or bill insurance, or any government sponsored insurance plans?
No, Wise-Minded Wellness is a private, independent, NP run practice, therefore we do not accept or bill: Medicare, Medicaid, Badger Care, or participate in any other insurance plans.
We can provide you with a Super Bill (insurance ready documentation/receipt), for you to submit for possible reimbursement, if you choose. All services utilize standard billing codes. We recommend reviewing your out of network benefits – prior to your appointment, if you have questions regarding reimbursement.
What form of payment do you accept?
Wise-Minded Wellness is a cash-based practice. All payments are due at the time of service.
We accept cash, checks and credit/debit cards. Returned, cancelled or stopped checks will add a $35 service fee. We also accept HSA and FSA (health or flexible savings account) cards but recommend reviewing what is reimbursable prior to your appointment.
All appointments require a valid credit card on file.
Can Wise-Minded Wellness serve as my Primary Care Provider?
No, Wise-Minded Wellness does not offer routine primary care type visits. It is imperative you have and maintain your regular Primary Care Provider (PCP) for all acute, chronic illnesses, as well as for your ongoing health maintenance exams.
